Do you need a modded ps2 for it to work ?

Yes and No, the disc will work regardless but with a phat ps2 you will need to take off the disc cover to get the slider in and with the ps2 silm you will need to do some sort of modification.

Since it reads ntsc games for a pal ps2. does it still apply to ntsc ps1 games on a pal ps2?

I think ntsc games run anyway without swap magic. If not try to use a backip copy of game in pal format
