They are all shipped empty without butane

Q: is there a way to adjust the flame height?

Yes there should be a little turning dial next to the hole where the butane is filled. It can be moved left or right for flame height adjustment.

Q: Does it fit thyrm case?

yes. but it doesn't fit the Zippo case. A Z-plus fits both. a zippo butane insert does not work in a thyrm case.
