Yes, you should get a screw for every hole. The T screw is used to lock the picture in place.

I don’t know if I am really giving you the answer you need but it seems to me that the 5 x 2 x 0.5 inches is referring to 5” is the length of that long flat piece ( although it matters very little how long that piece is except if you need a certain length to reach the place the screw gets put in). I believe the 2” and 0.5” is the dimension for the rectangular piece they show on the left. As far as the size of the screw. I don’t think any of those measurements refer to the screw size.
